Shifa Clinic is a primary care and family medicine practice operated by Vcare Family Practice Inc., serving Vernon-Rockville, Connecticut. This policy explains how we handle information collected through this website and how your health information is protected in our practice.
Shifa Clinic is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Your medical records and other protected health information are safeguarded under the HIPAA Privacy Rule and applicable Connecticut law. Information you give us. When you use the contact form we collect your name, subject, preferred location, phone number, email address and the message you write.
Information collected automatically. Our web host records standard server logs, which may include your IP address, browser type, the pages you visit and the time of your visit.
Cookies. This site uses only the essential cookies needed for it to work. We do not use advertising cookies.
The contact form is not a secure channel and should not be used to send symptoms, test results, medication lists or any other medical detail. For anything clinical, please use the patient portal or call us at (860) 375-8440. If this is an emergency, call 911.
We use the information you provide to answer your enquiry, to schedule and confirm appointments, to keep the website working properly and secure, and to meet our legal and regulatory obligations. We do not sell or rent your information to anyone.
We share information only with the service providers who help us run the practice — for example our website host, email provider and patient portal provider — and only to the extent needed to deliver that service. Where the information is protected health information, those providers are bound by business associate agreements. We may also disclose information when required by law or to protect the safety of a patient or the public.
Website enquiries are kept only as long as needed to deal with them and to keep a record of our correspondence. Medical records are retained for the periods required by Connecticut law and by our professional obligations.
Under HIPAA you have the right to see and get a copy of your medical record, to ask us to correct it, to ask for limits on how we use or share it, to ask for a list of certain disclosures, to receive a paper copy of our Notice of Privacy Practices, and to file a complaint if you believe your privacy rights have been violated. Filing a complaint will never affect the care you receive.
This website is not directed at children and we do not knowingly collect information from children through it. We do of course care for children as patients, and a parent or guardian manages their records.
